What is the process of Claim:

Step 1:

Reach out to either of the options within 48 hours with intimation of claim:

Step 2: 

Courier below shared list of documents 

  1. Completely filled & signed Claim Intimation form (available on our website)
  2. Copy of Death Certificate (issued by the local health and medical authority)
  3. Certificate of Insurance (In - forced, issued at the inception / renewal whichever is applicable. 
  4. Copy of Age proof of Life Assured 
  5. Copy of ID & Address proof of nominee, 
  6. Copy of Bank Passbook / cancelled cheque of nominee  
  7. Copies of First Information Report & Post Mortem Report, duly attested by police officials, in case of unnatural deaths including accidents, murder, suicide etc.

List of documents to process the Death claim:  

**Any other document or information that we may need to process the claim depending on the cause or nature of the claim.

**All copies to be self-attested by the Master Policy Holder

Note - Approval or rejection of the claim will be basis the investigation & documentary evidence provided to support and in case of but not limited to the below scenarios:

  1. If the life assured and the customer taking the ride are two different individuals.
  2. If there is a minor mis-match in the name of the life assured on the Certificate of Insurance and the above mentioned claim documents provided.
  3. If there is a major mis-match in the name of the life assured on the Certificate of Insurance and the above mentioned claim documents provided.
